Me too. I have a few guesses, but none seems like a hole, rather just hard to pull off.
Like mana based regenfighting (higly poisonous warlord, well built gorgon with cydstepp + mystic balance), lifesteal based regenfighting - seriously, if blood hunger didn't erode resistances this would be one of the most broken thing ever - (dracul pala).
I also used to believe protection stacking didn't exist, but EM with platemail + taurog's shield can yield great results. Have you tried a panzerrouge? Give an orc rouge these stuff with RBS and regenfight whatever you want. Entanglement will make you 1st strike as well :)

Oh, I found something that may fit in the "hole". Max HP decreaseing. Orb of Zot does it, and nothing else, and it doesn't quite combo with anything in the game yet. But I could imagine it being an ability of a class, who removed X max HP/hit/level, and that would consume blackspace nicely if you overused it. But I'm probably off with that.

We all miss infinite burning caps...

Sticking lifesteal on a maxed out resist monk (or pally) is still one of the most effective strategies. I would also assume that is why it is hated by the one god who gives DR (a way to gain even more resists than maxed resists) and the one who gives solid MR (GG, although he is inferior to binlor in this aspect, he is debatably better for pallys because of the mana bonus from enlightenment). Really, lifesteal is one of the most dangerous mechanics. Just think of how many broken iterations of vampires there were.

That dwarf change didn't last long at all. Do you guys actually play dwarves though? They still seem kind of weak, IMO, outside of rogue (which kind of prohibits them being buffed). It's a shame they can't be buffed.

The extra class really must be something out there. Like the alpha transmuter (speaking of broken stuff). It really had a totally different regeneration strategy which was debatably more nuanced.
We made an expansion and it is awesome. Really, you should check it out, especially if you're looking for some extra challenge.

The oldschool Healthmonster is still very viable despite the GG nerfs late in beta. That's about my only Dwarf preferred combo really (Rogues I'd rather just have more base damage).

For those who don't know (I've talked about it a lot because it's more or less my only interesting contribution to the forums on the strategy side  ), it's a Dwarf Paladin of GG with Fire Heart. Three good sources of % healing (Fire Heart, Protection, Potions), a lot of additional health, some bonus mana and damage, a strong start, all debuffs are largely unimportant to you, spike away.

It's awesome at Dragon Isles and Naga City, works alright in some other tough scenarios.

Edit: also, Lujo, best of luck with that shit - hope it sorts itself to your satisfaction.

Wow, really cool. I always believed that Burndayraz had this secondary ability from the get go, nice to see it's history. Were there any other glyphs that got altered similarly?

Oh and it reminded me about a signpost msg mentioning a 3rd heart that was lost forever. Is that the original Fire Heart, or something else?

I know I'm asking a bunch of unrelated questions for this topic, but DD history is so interesting and there's no topic about it (at least one that I know of). :P

Some things from the classic dd were in the beta but were so broken (like the transmuter) that they changed it completly.
You can check the classic wiki and some things may be in the beta.
