I've noticed that quite a few players talk about including Patches amongst preparations.

I've never really seen his effects spelled out, and I always seem to get the wrong side of the dice when i take him along.

Do you come out better or worse off in the long run if you use him?

As long as you have HALPMEH, and you convert after you use your burn salve, you're fine.

I suppose you would want him for the occasional resist buff? or does he grant awesomer things?

Resists, max mana, and extra gold are the three big ones. Overall he's a bit of a gamble, but he pays off enormously in the long-run.
