There seems to be an inconsistency in how Chaos Avatar seems to work. Below level 10 it seems to increase level without giving XP, whereas as of level 10 it seems to give XP. Not sure if this qualifies as a bug though, as it only seems to impact the victory screen XP metric.

Good/bad news. Got it to crash when I hit a cursed monster again. http://i.imgur.com/bdV0EH6.jpg
Once it happens it happens every time even if I start a new dungeon. On the weird side, I closed DD and tried again... No crash.

I hate temperamental bugs.

One solution would be to make Reflex Potion grant temporary Deathgaze immunity. It's not really a bug, but it's definitely an obscure edge case that ended a Darvin streak. Massive props just for being able to solve the puzzle.

The "Statue" Enemy that drops the Ritual Scroll in one of the northern Subdungeons appears to be level 1 - but Swift Hands doesn't work against him, even if you're level 10. I guess that he's either like a plant, or being immune to physical attacks screws with the ability, but it looks weird in any case.
