Hm. The Wiki says Ick Swamp is unlocked by beating Southern Swamp - which I've done with four classes, including all available quests. No Ick Swamp, though. :(

I DID find Mischief in Hexx. Just not sure what combination of abilities I need to play things by his rules. One of his forms has retaliate fireball, and a few are level 10, so I can't Assassin cheap-shot past him either. Best I can think of is an Orc Rogue with a Crystal Ball to fireball the forms without retaliate.

I'm pretty sure it's based on your level, and he levels 3 or so levels throughout the fight. Start as a level 1 wizard. Find Mischief, level up to 3 or 4 and destroy him.
We made an expansion and it is awesome. Really, you should check it out, especially if you're looking for some extra challenge.

I did an unlock strategy guide of sorts a while back. Might answer some of your questions.

I should add that some of the details have changed since I made that. The way that banks give money to adventurers changed, making the bank all upside with no downside. Tikki Tooki and Earthmother switched places in unlock order, and the criterion for opening the directional quests are slightly different now. Also, that was when challenges didn't give any money. If I were to do this again, picking off bronze and easy silvers would have been a big part of my planning.
