I'd wonder if it stacks... would be good to know. At very minimum, WEYTWUT will work.

And I always thought you can't first strike higher level stuff with Platemail on :P
Turns out all you need is GETINDARE + WEYTWUT.
Somebody spilled raspberry juice all over the place!

2) Monster with First Strike (including monster's death gaze)

Hmm... that could be misleading. Not every monster with death gaze has first strike. Death Gaze always activates with the same speed as the monster's regular attack. Gorgons just happen to have both first strike and death gaze.
