The PQI will appear in a somewhat randomly chosen dungeon (selection is heavily weighted towards dungeons with lower completion) asking you to defeat it with a particular race and class, while earning a particular badge. If possible, it will choose a class and badge that you don't yet have credit for. The reward is always 500 gold. When you complete the Perpetual Questing Initiative, it will reappear in a new dungeon (this may also happen when you complete any dungeon, or quit the game).
